Communicable Disease forms in the 2009 CD Manual support the newly revised NC EDSS disease models. NC EDSS revised data entry screens will be available August 10, 2009.

DISCLAIMER: The communicable disease reporting forms available on this website are the official reporting tools for the North Carolina Division of Public Health. Whereas the forms are functional, they are still considered draft documents until NCEDSS has completed implementation for reporting of all diseases. Roll out of reporting for the three remaining diseases is expected to be complete by January 2010.
